How to Do ERP for Pure OCD

What is Pure OCD?

Pure OCD, often called “Pure O,” is characterized by intrusive, unwanted thoughts — obsessions — without visible compulsions. But in practice, most people with Pure OCD do perform compulsions. They’re just mental ones: thought suppression, checking, analyzing, and more.

Understanding ERP

ERP is a structured approach designed to help you face your fears and reduce compulsive behaviors. It might seem hard to apply when the compulsions are mental, but the principles are the same.

Common mental compulsions

  • Thought suppression — trying to forcefully push intrusive thoughts away.
  • Thought checking — repeatedly revisiting thoughts to analyze their significance.
  • Praying — prayer can be a healthy spiritual practice, but using it compulsively to neutralize anxiety reinforces the loop.
  • Counting — mentally counting to avoid or dismiss obsessions.

Steps to conduct ERP for Pure OCD

  • Identify obsessions and compulsions. Identifying the mental compulsions is what makes the difference here.
  • Experience the thoughts. Allow intrusive thoughts to be present without attempting to neutralize or avoid them.
  • Prevent compulsive responses. Recognize when you’re engaging in a compulsion — analyzing, praying compulsively, any other mental act — and resist the urge.
  • Habituate to the anxiety. Over time, exposure to these thoughts without engaging in compulsions reduces the anxiety they provoke.

An example: existential obsessions

Consider someone obsessed with existential thoughts like “what if God doesn’t exist?” They might avoid situations that trigger the thought, or engage in compulsive prayer.

  • Exposure: allow the thought to be present without engaging in rituals or compulsive prayer.
  • Response prevention: distinguish between faith-based prayer and compulsive prayer, and refrain from praying to neutralize the anxiety around the thought.
