Pure O OCD: Understanding and Treating Pure Obsessional OCD
July 17, 2017
What is Pure O OCD?
Despite its name, Pure O OCD is essentially just a subtype of Obsessive-Compulsive Disorder (OCD) characterized by intrusive and taboo obsessions. Often these obsessions revolve around themes of harm, sexual content, or other distressing thoughts.
The myth about Pure O
There’s a common misconception that Pure O OCD involves only obsessions without any compulsions. The OCD community has clarified that compulsions are present in Pure O — they’re just more subtle. These compulsions often take the form of mental rituals, such as praying silently, counting, or ruminating.
The OCD cycle in Pure O
Understanding the OCD cycle is crucial for anyone looking to overcome Pure O. The cycle starts with an obsessive thought triggering an anxiety spike. To relieve that anxiety, a person engages in a compulsive behavior, which perpetuates the cycle. Identifying those compulsive behaviors, especially when they’re subtle and mental, is essential for treatment.
Common mental compulsions:
- Praying silently — repeated silent prayers to neutralize distress.
- Counting — mentally counting to fend off anxiety.
- Ruminating — engaging in internal debates to gain certainty (for example, questioning one’s sexual orientation).
Treating Pure O
The good news is that Pure O OCD is treated the same way as any other form of OCD. The most effective treatment method is Exposure and Response Prevention (ERP):
- Exposure — gradually exposing yourself to the fear or obsessive thought.
- Response prevention — preventing yourself from engaging in the compulsive behavior that follows the obsession.
By practicing ERP, you can allow yourself to experience fears without resorting to compulsive behaviors, breaking the OCD cycle. Incorporating mindfulness and acceptance of uncertainty can further aid in managing Pure O.
My own experience
Throughout my journey I struggled with various forms of Pure O OCD, but with consistent effort and applying ERP, I’ve successfully kept OCD out of my life for many years. Overcoming Pure O is entirely possible.
The bottom line
Pure O OCD is a manageable condition just like any other form of OCD. It involves both obsessions and compulsions, though the compulsions may be more subtle and mental. With the appropriate approach — particularly ERP — you can overcome it and lead a fulfilling life.
